It is advised that you should provide your own personal injury insurance for participation in the Badger State Games. The Wisconsin Sports Development Corporation will not be held liable for any injury incurred from participation in or association with the Badger State Games.

It is in your best interest to call and confirm the receipt of these forms. The office number is 608.226.4780. Scholarship funds are intended for youth upto age 18.
